Instructions
Applicants should bring the following;
- Original Birth Certificate or copy
- National Identity Card or copy if available
- Current Passport – If lost, a copy of the Passport.
- Three (03)color photographs (size 3.5 cm x 4.5 cm) taken within last 3 months;
*Showing full face with both ears visible and taken without hat, jackets, spectacles &earrings - by male applicants
- Alien Registration card (Korean visa card) – In case the applicant has a valid visa
Note: If the applicant is a dual citizenship holder, the foreign passport as well as the dual citizenship certificate should be produced.
For the applicants who lost/destroyed their current passport
For the applicants who have not legal visa status in the Republic of Korea

- Original Sri Lankan Birth Certificate of the child
- If born abroad, Sri Lankan Citizenship Certificate
- Three (03) color photographs (size 3.5 cm x 4.5 cm) taken within 01 month; Showing full face with eyes opened
- Letters of Consent by both parents
- Original Passports of both parents
- Original (Sinhala/Tamil) Marriage Certificate of the parents

Change of Name or Profession
Applicants should bring the following;
- Valid Passport
- Original (Sinhala/Tamil) Birth Certificate
- If change in name: An Affidavit to be given at the Embassy
If changing the maiden name: Original (Sinhala/Tamil) Marriage Certificate
- If the Profession to be included/changed:
Relevant Professional/Employment Certificates counter certified by the Ministry of Foreign Affairs in Sri Lanka or Korea

Note : Change of name or profession is made only after obtaining the approval from the Department of Immigration & Emigration of Sri Lanka.
Registration of Birth and Granting Sri Lankan Citizenship
Applicants should bring the following;
- Korean Birth Certificate with English translation (If born in Korea)
- Parents’ original Birth Certificates (English translations are not accepted)
- Parents’ original Marriage Certificate (English translation is not accepted)
- Parents’ original Passports
- Declaration by father (If father is Korean, mother should give it)
- Korean Visa card (Alien Registration card)
- If the father had no valid visa at the time of the child's birth, he should submit an Affidavit in this regard.
Note : Normally, a birth should be registered within 01 year of the birth. Otherwise, approval of the Registrar General of Births, Marriages & Deaths should be obtained in order to register the birth.

Applicants should bring the following;
- Two (02) separate Affidavits (Unmarried Certificate) given by a parent of both parties in front of a lawyer/Justice of Peace to confirm the present civil status of both. This affidavit should include each parent’s no objection for the proposed marriage.
- If divorced, the Divorced Certificate
- If widowed, the Death Certificate of the late spouse and previous Marriage Certificate
- If adopted, the Adoption Certificate
- Original Birth Certificates of both(English translation are not accepted)
- Valid Passports of both
- Two (02) witnesses with valid Sri Lankan Passports

Note :
- All above documents except Passports should be duly authenticated by the Consular Affairs Division of the Ministry of Foreign Affairs of Sri Lanka.
- All names, passport numbers, Identity card numbers should be correctly mentioned in the Unmarried Certificate.
Relation, friend or any other person who has an eyewitness to the death should inform about the death to the Embassy.
Required documents for the registration;
- Death Certificate issued by the Doctor (with an English translation)
- Certificate issued by the hospital handing over the Human Remains (with an English translation)
- Other related documents of the death issued by the Korean authorities (with English translations)
- Passport, Birth Certificate and other identification documents of the deceased.
- Declarant's Passport or any other identification document

Driving License
- Original Sri Lankan Driving License
- Electronically attested extraction of Driving License by the Consular Affairs Division of the Ministry of Foreign Affairs of Sri Lanka along with the original document
- The applicant's full name (as indicated in the passport) should be correctly mentioned in the Driving License or its extraction. Otherwise issuance of Korean Driving License would be rejected.

Sri Lanka Driving Licenses
Applicants should bring the following.
- Current Sri Lanka Driving License
- Medical Certificate
- Valid Passport
- Alien Registration Card
- National Identity Card (If available)
- Two (02) Black & White photographs (size 3.5 cm x 4.5 cm)
- Copy of the Complaint made at the relevant Police Station (if the Current Driving License is lost.)
